AN NHS community learning disabilities team is inviting people to find out more about what their specialised service has to offer.

The team, based in Trafford, is part of Cheshire and Wirral Partnership NHS Foundation Trust (CWP). They provide services for adults from the age of 18, who have a learning disability and live in Trafford.

The team is made up of health professionals and social workers who specialise in working with people with learning disabilities. The team works together to plan and provide a range of services for people who may be experiencing additional needs. The team also provides specialist information, advice and training for family, carers and support staff.

Avril Devaney, director of nursing and therapies says, “Our aim is to provide health and social care for adults with learning disabilities, their families and carers who live in Trafford with a focus on rights, independence, choice and inclusion.

“We know that people with a learning disability can suffer much poorer physical health than the rest of the population. Joining up with other professionals allows us to deliver a seamless service to improve the health and well-being of people we serve in our communities, supporting them to lead full lives within the local community.
